Forehead reduction surgery cost: $5,000–$15,000 in the US, £7,000–£9,000 in the UK, $2,500–$4,500 in Turkey. Results, risks and hair transplant option.
Forehead reduction surgery (hairline lowering) costs $5,000 to $15,000 in the US, averaging about $7,600 to $9,000; £7,000 to £9,000 in the UK; and $2,500 to $4,500 in Turkey, often with hospital, transfers and hotel included. Insurance does not cover it. A systematic review of 882 patients (meta-analysis of 801) found a mean lowering of 1.6 cm with a pooled complication rate of 1% or less; a 650-patient series reported 2.29 cm average excision (28% of forehead height), with folliculitis (20.5%) and temporary shedding (15.1%) as the most common problems. The change is long-lasting, though the hairline can still recede with hair loss.

Forehead reduction, also called hairline lowering, removes a strip of forehead skin just in front of the hairline and moves the hair-bearing scalp forward. The incision follows the hairline so the scar can sit at its edge. It is aimed at people whose forehead is tall in proportion to the rest of the face.

A systematic review of 882 patients (meta-analysis of 801) found a mean reduction of 1.6 cm (95% confidence interval 1.4 to 1.8 cm), with a pooled complication rate of 1% or less (Facial Plast Surg Aesthet Med, 2022). A newer series of 650 patients reported an average skin excision of 2.29 cm, about 28% of forehead height, and rated all scars as good quality (Aesthetic Plast Surg, 2025).
The change is long-lasting because the removed skin does not grow back, though the hairline can still recede with hair loss. The hairline can still recede later if you develop hair loss, so it is worth reviewing any family or personal history of hair loss first — see our guide to the stages of hair loss.
| Forehead reduction complication (650-patient series) | Cases | Rate |
|---|---|---|
| Folliculitis (inflamed hair follicles) | 133 | 20.5% |
| Temporary hair shedding (postoperative effluvium) | 98 | 15.1% |
| Wound opening (dehiscence) | 14 | 2.2% |
| Seroma | 6 | 0.9% |
| Skin necrosis | 1 | 0.2% |
| Alopecia (hair loss) | 1 | 0.2% |
Rates calculated from case counts in the 650-patient series. Men had less shedding than women. The meta-analysis lists temporary and permanent hair loss, unacceptable scarring, persistent numbness or tingling, and haematoma as the complications reported across studies. The 650-patient authors also note that reductions above about 2.3 cm appeared to raise the chance of needing a scar revision, though this was not statistically confirmed.
Surgery lowers the whole hairline in one step and gives a fixed result within one operation. A hair transplant moves follicles forward instead and can also fill the temples; the 650-patient authors note that correcting temple recession is one of the limitations to discuss before forehead reduction. The number of grafts drives the price — see how many hair grafts you need and our hair transplant for women page.
You cannot shrink forehead skin or bone without surgery. A hairstyle or a hairline hair transplant can make the forehead look shorter, and some people combine approaches. If hooded brows rather than forehead height are the concern, an eyebrow lift is a different operation, listed by Livist Medical at €2,230 (about $2,498 at 1 EUR = 1.12 USD).

Results vary from person to person and are not guaranteed.
